INSTRUCTIONS FOR COMPLETING APPLICATION
FOR ENGLISH AS A SECOND LANGUAGE
PACKAGE
Required information for processing the application package
Please complete the information on the application form. Print clearly. Answer all questions. If the question is not applicable, please write n/a.
Address:
According to U.S. Government regulations, international students are required to complete their current international address in order to receive an I-20/DS-2019 form. An agency address is not acceptable by U.S. Government regulations.
Bank Statement:
An ORIGINAL bank statement is required (issued by the bank) for the most recent six months indicating a minimum $16,300.00 USD balance ($4,100 USD per 9 week term). The bank statement should be issued on original letterhead of the bank with its seal/stamp (not Savings Department’s seal/stamp) and should reflect bank transactions. The letterhead of the bank should include address, e-mail, web address, phone number, fax number of the bank, etc. for verification purposes. We will not accept any copy of the bank statement sent to us as a fax copy, e-mail attachment, Xerox copy, or a copy certified by an agency. If the bank statement is not in English, please make sure you also provide a translation.
Sponsor Letter:
If the bank statement is not in the applicant’s name, an original sponsor letter should be provided as well. The person whose name is on the bank statement should provide a letter stating the relationship to the applicant and that he/she is willing to sponsor the applicant while he/she is at Troy University. The letter should be signed and dated. If the sponsor letter is not in English, please make sure you also provide the translation.
If the sponsor is a US citizen/Green Card holder, he/she should fill out form I-134 (Affidavit of Support), and then have it notarized and send the original to our office. The I-134 can be found at http://www.uscis.gov/files/form/I-134.pdf
